K tomu odkazuje i název hry: v brzkých ranních hodinách totiž páchá sebevraždu největší část takto nemocných lidí. Sama Kane prý se právě ve 4.48 nejčastěji probouzela do silně depresivních stavů. Sarah Kane po dokončení této hry spáchala sebevraždu.
