Kniha vznikla z Foucaultových přednášek na College de France v akademickém roce 1975–1976 a znamená důležitý okamžik ve vývoji jeho vlastních badatelských zájmů. Foucault zkoumá, jak se rodí v novodobém historickém diskurzu téma vnitřního nebezpečí, jež údajně ohrožuje civilizovanou společnost, ať už v podobě nepřítele rasového, třídního nebo jiného, zejména takového, který ohrožuje její zdraví, který představuje cosi zlověstně nemocného na jinak „zdravém těle“ společnosti.
